NEW PATIENT INFORMATION
PAPERWORK
Please take the time to fill out all the paperwork before your visit. This helps us efficiently use the time provided for your visit to focus on finding the problems and providing your options for care. The paperwork may be extensive, make sure you provide yourself enough time to complete it appropriately before your visit.
PATIENT VISITS
- $150/hr billed in 30 min increments
- A new patient visit consists of a consultation, examinations, treatment when appropriate, and recommendations for further testing or treatments.
- This visit usually lasts 1.5-2 hours.
- $75-$150
- Each visit after the initial visit is considered a follow up visit. During a follow up visit we will re-evaluate the findings from the previous visits, address any new findings, and check nutrition or supplements as necessary.
- This visit lasts between 30-60 min.
All fees are due at the time of service. Dr. Herbert is not a provider for any insurance company. If you plan to use your insurance, Dr. Herbert is able to submit the information to your insurance company electronically for reimbursement.
We accept all major credit cards as well as cash and checks for payment.
We do not accept assignment with Medicare, which means the patient pays at the time of the visit and we will submit to Medicare on the patient’s behalf. Medicare will then reimburse the patient according to their standards.
LOCATION
We are located upstairs in Gordy’s Plaza. Enter through the front doors below the flashing sign in the front parking lot along highway 53. Go up the stairs, turn right, go through the door, turn right again and we are at the end of the hall on the right. If you are taking the elevator, when you exit turn right and we are down the hall on the right.

Our office hours are continually changing due to demand for appointments and ability to find child care. Please contact the office to schedule and for the most up to date hours.
